Subject Access Requests (SARs) and Non-NHS Work

In addition to NHS services, we are able to provide a range of non-NHS services, including Subject Access Requests (SARs), medical reports, medical certificates, GP letters and other forms.

Please note that Subject Access Requests are normally provided free of charge. Other non-NHS services will usually incur a fee.


Subject Access Request (SAR)

If you wish to have a copy of all or part of your GP medical record you may request this under a Subject Access Request (SAR).

About SARs

If you wish to have a copy of all or part of your GP medical record you may request this under a Subject Access Request (SAR). The completed report will be released directly to the patient*, for you as the patient to share with whomever you choose.

This is a factual copy of your records. It could also be a summary of your immunisations or medical conditions and medications.

There is no charge for this service and requests should be completed with one calendar month.

*If you are a solicitor making a request for information please submit your request via our dedicated email attaching a pdf letter to support this request.

We will need to verify your identity in order to protect your personal data.

We have one calendar month in which to respond to your SAR. This time may be extended by up to a further two months, if your request is complex or we have received several individual rights’ requests from you.

We may charge a ’reasonable fee’ for the administrative costs of complying with a request if it is manifestly unfounded or excessive, or if an individual requests further copies of their data.

Non-NHS Work

We offer a range of non-NHS work (sometimes called private GP services), including medical reports, GP letters, medical certificates, occupational health reports, fitness to work documentation, DVLA medicals, taxi medicals and other medical forms.

These services are not covered by the NHS and therefore incur a fee. Fees vary depending on the type of report, certificate, form or assessment requested and must be paid in advance before work can begin.

All private and non-NHS work are provided at the discretion of the practice and requests may be declined if we are unable to provide the service requested or consider it unsuitable. If you cannot find the medical report, GP letter, certificate or form you require in the lists below, please contact us to enquire.

Medical Reports, Medical Certificates and Non-NHS Work We Can Offer

    • Armed Forces Medical / Ministry of Defence recruitment form (E.g. Army, Royal Navy, RAF, Police)
    • Employment/pre-employment medicals and paperwork (e.g. Police, NMC validation (Review of medical notes and factual reports only. No opinion base will be provided)
    • HGV Medical for DVLA (D4)
    • Bus Driver Medical for DVLA (D4)
    • Taxi Driving Medical (Local Authority Paperwork)
    • Ambulance Driving Medical (Paperwork must be reviewed prior)
    • Health and Safety Executive (HSE) Section 33 Report
    • Sports medical certificates / Fitness to participate (EG Cross Fit Games)
    • Motorsport Medical (requires ECG, urine test, up to date weight and blood pressure which we are able to provide)
    • Holiday Cancellation Forms (Date of policy, data of cancellation and clear reasons for cancellation must be provided)
    • Statement of diagnosis and medication for travel
    • Adoption and fostering medicals and paperwork
    • Medical paperwork (e.g. Bupa, Vitality)
    • Shotgun or Firearms Medical Report (For issuing authority)
    • Overseas employment (e.g. Camp America, resort work)
    • Letter of support / To whom it may concern (E.g. Exam concessions, coursework extensions, court obligations)
    • Student Finance England (SFE) / Disabled Students Allowances (DSA)
    • Home Office (Factual Report)
    • NHS Pension applicant confirmation

We may also be able to process work from an external requestor; they will provide the paperwork for this request directly. This may include: Insurance request, Solicitor Reports, DSS / Benefits e.g. PIP, Probation Service, DVLA and Occupational Health factual reports for recruitment (e.g. Military, police)

Once the practice has received all the required information, including necessary consents, your request will be processed and an invoice (as required) may be issued.

You should allow 4-8 weeks for Non-NHS Work requests. At times this may take longer due to NHS patient care taking priority. Please ensure all Non-NHS Work requests are submitted in plenty of time for any required deadlines; we cannot expedite requests nor meet urgent deadlines.

You can make a Non-NHS Work request via our Online Consultation Service; you will be asked to confirm you have read all the relevant data within this page. You will not be able to request a service that is not available.

Requests we are unable to offer

We are unable to process the following requests:

    • Aviation Medical (Recreational or professional)
    • Diving Medical (Recreational or professional)
    • Health examinations for  Immigration and citizenship (e.g. Australia)
    • Identity or address verification
    • Passport or Driving licence certification
    • Fit to fly documentation
    • Application support/GP letter of opinion e.g. housing applications, bus passes, prognosis

This list contains some examples and is not exhaustive. We reserve the right to decline Non-NHS Work.

 

Please Note

Submitting a request is not an acceptance by the Practice that the work can be completed. If we, or medi2data, are unable to complete the Non-NHS Work request you will be informed as soon as possible.
