Appointments or Administrative Requests

We now use the same Total Triage method for all medical requests for the GP Team and all Administrative Requests.

Fastest Way to Contact Us
Use SystmConnect for the quickest service. Your request will be directed to the appropriate person, and we will contact you as soon as possible.

Alternative Contact Methods

By Phone: If you cannot use SystmConnect, you can call 01752 205 555. The call handler will ask you the same questions and complete the SystmConnect forms for you.

In Person: If you lack online or phone access, you can visit our counter. Be aware that the same SystmConnect forms will be used, privacy cannot be guaranteed, and staff may need to pause to assist other patients.


Form Processing
Once your SystmConnect forms are completed, they enter one of two queues:

Clinical Assessment: A senior clinician will review your form. Based on this assessment, you may be asked to provide a photo, supply additional information, be directed to a more appropriate service, receive details of the next steps, or be asked to come in to see a clinician, possibly on the same day.

Admin Assessment: These requests will either be addressed immediately or forwarded to the most appropriate person for handling.


 

Cancel your appointment

It is important that you inform the reception staff please if you are unable to attend your appointment, this will allow that appointment to be offered to another patient.

This can be done by one of the following methods:

  • Submit the form on Using SystmOnline here. (Please use ‘Other admin request’ at bottom right)
  • Via our online services
  • Via NHS App
  • Call the practice on 01752 205555
  • Visit Reception at either site

If you fail to notify the Practice that you are unable to attend, you will be sent a letter informing you that you did not attend your appointment. Persistent defaulters may be removed from the list.


 

Home Visits

To ensure high-quality service, we need your help in using our limited clinical capacity effectively.

Home visits take up to 45 minutes of a GP’s time, with up to 8 requests per day. We triage visits to ensure a clinician’s attendance makes a significant difference to the patient’s health, often achievable in other ways.

Please notify us by 10:00am if you or someone you care for needs a home visit. This helps us plan our daily workload. Clearly state the problem, its urgency, and provide contact details for a follow-up call. For life-threatening issues, dial 999.

We will arrange a visit when it is the only way to address the patient’s needs safely and effectively.
