Sick (fit) notes
Self Certification Sick/Fit Note for less than 7 days
If you have been ill for 7 days or less you do not need to speak to a Doctor. You can complete a Self-Certification form yourself, which can be found here. We are not able to provide Doctors notes if you are absent from work due to illness for less than 7 days.
Doctor’s Sick/Fit Note for more than 7 days
If you have been ill for more than 7 days you will need a doctor’s certificate. These certificates are called ‘Statement of Fitness to Work’ or ‘Med3’. The surgery will advise you whether you require an appointment.
If you have already spoken to your Doctor about your current illness and they have confirmed a Med3 has been issued, this will be sent to you electronically via text message or email. If you do not have access to receive this electronically then a paper Med3 will be provided for you, or a representative, to collect from the Surgery.
Requesting an additional Sick/Fit Note
If you have already had a Statement of Fitness to Work for this illness, your doctor may not need to speak to you to be able to issue a further Med3. Additional Med3’s will be sent to you electronically.
