Primary Care Network (PCN)

A Primary Care Network (PCN) is a group of local GP practices working together to provide better, more joined-up healthcare for their community. PCNs are part of the NHS Long Term Plan, funded nationally to support and grow general practice, helping people stay well at home and in their communities.

You remain registered with your own GP and continue to book appointments and access services as usual through your surgery.

Drake Medical Alliance PCN consists of:

  • Knowle House & Tamerton Surgery
  • Lisson Grove & Woolwell Medical Centres
  • Roborough Surgery
  • North Road West Medical Centre
  • Wycliffe Surgery

Together, we care for around 61,000 patients across Plymouth.

The PCN is led by a Clinical Director (a practising GP) and supported by a PCN Manager. This team brings local practices together to improve care while gaining the benefits of working at scale.

Through your PCN, you may have access to a wider team of healthcare professionals such as

  • Clinical pharmacists
  • First contact physiotherapists
  • Mental health workers
  • Social prescribing link workers

This wider team helps ensure you see the right person for your needs, while freeing up GP time for those who need it most.
