Register with our practice

We are asking all our patients to use our online registration forms (below) to save trips to the surgery. Please complete all fields to avoid any delay in your registration. Please use our boundary checker to check that your address is covered by us.

If you are not able to access the form, a paper copy can be collected from the Practice but please call us first to make sure they are ready for you. Please also let us know if you are housebound.

Please note that we cannot accept postal registrations.

For information for overseas visitors to the UK requiring access to medical treatment please click here.

If you leave our practice boundary you will need to reregister with a GP closer to your new residence. We do not accept ‘out of area registrations‘.

We are one practice with two sites, once registered you can access services at either site.
Please choose the site you are likely to use the most to help us plan.

Catchment Area (Practice Boundary)

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.

Named GP

In line with our contractual requirements all of our patients, as of the 30th June 2015, will be allocated a named and accountable GP.

What Does Accountable Mean?

Your named accountable GP will oversee the co-ordination of your care amongst the other health professionals.

Does this mean that my named accountable GP will take 24hr responsibility for my care?

No, the named accountable GP will not take on 24hr responsibility for your care, your named GP will purely oversee the care that you receive.

Does this mean that I have the right to see my named GP every time I book an appointment?

No, a patient can express who they would like to see, however, this may not always be possible due to appointment availability with that GP.

Can patients still see any GP they wish?

Yes, a patient can continue to see any GP they wish as they currently do.

Will I be informed in writing who my accountable named GP is?

No, your practice does not have to inform you in writing. If you would like to know who your named GP is then please contact the practice.
